Need Simple PHP Code to Search a Data from MY SQL

I have a MySQL Database with theses five columns

1. Unique ID
2. Name
3. Exam
4. Score
5. Global Rank
6. Remarks

I want a PHP code where I will input unique ID and press ok then it will search the database and show the details like this in simple way

Unique ID :	XTY123
Name :	Name of the user from the database
Exam :	Exam name
Score :	Score in Number
Global Rank :	Global rank in Number
Remarks :	Remarks any

else it will show " No ID Found "

That's all
