What do they have to do every month? Why isn't it a one time job?

You are right that SEO work is a continuing process because you might be left behind by your site’s competitors when you stop doing SEO work on your site. If you have the time then you can do it yourself but for those who cannot spare time for doing SEO work on their site has to get an outsource or a freelancer who can do the SEO work for them. It is important to work on the site regularly particularly with the creation of contents.

It is an ongoing process - however if your website has less competition and specific to one particular geo location then try one time seo - Just go to google and type "one time seo" and search you will find some articles by me.

Because maybe you don't have time to do it yourself and don't want to lose your ranking? I mean why give them a work and not pay them?
Based on the comments above, it is an ongoing process. So basically yes, you need to pay them because they are working for you.
You need to boost the traffic of your website, if you can't do it yourself, you pay others to do it for you.

I have read in one discussion about a percentage scheme for paying the SEO specialist. He will be doing SEO work on your website regularly but the compensation will be a percentage of the profit that your website will get from the SEO work.
