This honestly depends on the site you're using. Alot of people create fake accounts to get other accounts "real" likes/followers/etc. Depending on whom you ask, no it's not safe, and yes it is. So use caution whenever you use them.
Their ethnics say you must not unfollow/unlike/etc, but hardly anyone follows them.
Only use social exchange for high counts, getting real likes/followers is very slim, and about 80% of all users there either use bots, use fake accounts, or unfollow/unlike/etc.
